• Issuance of a new PIN on request is done based on a personally signed Form for PIN delivery (form PIN-DEP), that is a personally signed request (letter) in which you ask for a delivery of a new PIN. The request is signed by the owner/account holder, that is, his legal representative or guardian. The form must be accompanied with a copy of a valid identification document and proof of payment to the giro account of SKDD (Central Depository and Clearing Company) at the HPB (Croatian Postal Bank) number 2390001-1100017198 with reference number 156-ACCOUNT CODE. Issuance of a new PIN is being charged Kn 50,00 from January 1 2009 according to the pricelist of the SKDD.

The form is to be submitted to the SKDD by mail or in person (investor or owner/holder of the account, legal representative or guardian) to the SKDD Customer Office, Heinzelova 62a, 10 002 Zagreb, pp 409, from Monday to Friday from 8 to 16.
